Making an Appointment

All appointments are arranged via email. I am a solo practice and do my own scheduling, so I have limited time for phone calls. Please fill out the form below with your name, contact info, and a brief description of your concerns. I will respond within 2 business days with available times and fees.

Important: Please read the New Patient FAQ before scheduling. New patient evaluations require a signed evaluation policy and deposit within 7 days of scheduling. Required forms are found on the Office Forms page.
